"What happens to my Medicare in Bali?"
The short answer is that it keeps existing for a while and does you no good while you are there. That sounds glib, but the distinction between eligibility and access is the whole thing, and it is where people get caught.
How long eligibility lasts
Australian citizens remain eligible for five years from the date they first leave. The clock starts at departure, not at the point you decide the move is permanent.
Permanent residents who are not citizens have a much shorter window. More than 12 months overseas and the enrolment ends.
The three things that follow

You cannot use Medicare from outside Australia. Eligibility is not access. There is no reimbursement arrangement for a consultation in Sanur, a scan in Denpasar, or a hospital admission anywhere in Indonesia.

Indonesia has no Reciprocal Health Care Agreement with Australia. Australia has agreements with eleven countries, and Indonesia is not one of them. This is the point at which international health insurance stops being optional.

If you return after more than five years, you re-enrol. It is done through myGov, the documents do not need to be certified, and you need to be in Australia and able to show you have returned to live. If you re-enrol and later go overseas again, the five-year clock resets.
One practical tip that costs nothing: renew your Medicare card before you leave, so it carries a full period of validity rather than expiring while you are away.
What this means in Sanur
Sanur is a good place to carry this risk, which is part of why Australians concentrate there.
The Bali International Hospital is in Sanur, inside the Sanur health special economic zone, and has been operating since 2025. BIMC and Kasih Ibu are both around 15 to 20 minutes away. For anything routine, the question is which clinic you like rather than whether one exists.
Budget in the order of A$300 to A$900 a month for international cover — roughly IDR 3.75 to 11.25 million. That range is wide, and it is deliberately higher than the figures most Bali cost-of-living articles quote, because cover that genuinely includes evacuation and pre-existing conditions climbs steeply with age and with anything in your history. Treat it as a starting point and get quotes at your actual age before you build a budget around a number from an article, including this one. A quote is free and takes a morning.
What international cover actually needs to include
Two clauses matter more than the headline premium:
- Medical evacuation. If something happens that Bali cannot treat, you are going to Singapore or home, and that is not a cost you want to meet yourself.
- Pre-existing conditions. Read how the policy defines them and what waiting periods apply. This is where cheap policies get cheap.
Also check whether the policy covers you during trips back to Australia, because for your first five years you will have Medicare there anyway and you may be paying twice.
Two calls worth making early
Before you commit to a visa, a lease, or a purchase:
- A health insurer, for a real quote at your age with your actual history.
- A registered migration agent or Indonesian immigration, for your visa eligibility.
Those two answers turn a daydream into a decision. Everything else — which suburb, which villa, which floor plan — is easy by comparison.
